👁 Compare the movement of both vocal cords during quiet breathing and phonation. Observe whether both cords move and meet normally.

A 55-year-old man presents with a breathy, weak voice and occasional choking on liquids. Laryngoscopy is performed during quiet breathing and phonation. Which of the following is the most likely diagnosis?

Correct answer: A. Unilateral vocal cord palsy. One vocal cord moves normally while the other remains immobile in a paramedian position. The cords do not appose fully during phonation, leaving a persistent glottic gap. This pattern indicates unilateral recurrent laryngeal nerve palsy and explains the breathy voice and aspiration. Cricoarytenoid joint fixation can also produce an immobile cord, but passive mobility testing under anaesthesia is required to confirm mechanical fixation. Bilateral abductor palsy affects both cords and usually causes stridor. Superior laryngeal nerve palsy affects pitch, but the cord continues to abduct and adduct.

Correct answer: A. Bilateral diaphragmatic paralysis. The animation demonstrates paradoxical thoraco-abdominal movement. Normally, the thorax and abdomen move in the same direction during respiration. In bilateral diaphragmatic paralysis, the diaphragm fails to descend during inspiration. This produces abnormal inward movement of the abdomen and may cause significant respiratory difficulty.

In a circle breathing system, exhaled gas is passed through a canister containing granules before being rebreathed. Which of the following is the principal chemical reaction by which these granules remove carbon dioxide?

Correct answer: B. Neutralisation of carbonic acid by a base to form a carbonate and water. Carbon dioxide first dissolves in the water present in the granules and forms carbonic acid. The acid is then neutralised by bases in the absorbent, principally calcium hydroxide, producing calcium carbonate and water while releasing heat. It is a chemical neutralisation reaction and not adsorption, conversion to oxygen or condensation.
